The North Carolina Division of Public Health (DPH) uses Preventive Health and Health Services (PHHS) Block Grant funding to administer the Health Communities Program through the Chronic Disease and Injury (CDI) Section. The aim of this program is to reduce the burden of chronic disease and injury in North Carolina. Research shows that implementing policy and systems changes can result in positive behavior changes that decrease chronic diseases and injuries and improve health. Examples of such strategies include providing access to healthy foods and options for physical activity, promoting tobacco-free facilities, and providing evidence-based interventions for injury and violence prevention in communities.
County and district health departments are enabled to implement community-based interventions that address poor nutrition, physical inactivity, tobacco use, violence, and unintentional injury. Interventions should strive to provide opportunities for everyone in North Carolina to achieve their optimal level of health, regardless of race, ethnicity, gender, socioeconomic status, geographic location, education status, disability status or sexual orientation.
Diabetes Prevention Program (DPP)
Diabetes Prevention Program (DPP) has this program called Prevent T2, a 12-month program focuses on education, encouragement, and tools necessary to help individuals prevent or delay the onset of diabetes. Eligible participants can join Prevent T2 along with a trained Lifestyle Coach to share and learn about healthy eating habits, physical activity, and managing stress.
Program Eligibility:
o Be 18 years or older
o Have a body mass index (BMI) of 25 or higher (23 or higher if Asian American)
o Not previously diagnosed with Type 1 or Type 2 diabetes
o Not be pregnant
And have at least 1 of the following requirements:
o Hemoglobin A1C: 5.7 – 6.4%
o Fasting plasma glucose: 100 – 125 mg/dL
o Two-hour plasma glucose (after a 75g glucose load): 140 – 199 mg/dL
o Be previously diagnosed with gestational diabetes (diabetes during pregnancy)
o Received a high-risk result (score of 5 or higher) on the Prediabetes Risk Test
